Infamy and Fame points
It might be nice if DMs could award infamy or fame points, which NPC's could react to. Say you do a very minor quest to help people- you get 1 fame point. Or, you do a fairly impressive quest- that's 5 points. You saved the city/country/world? 10-15 points to gryffindor! If you do chaotic/evil actions you can get infamy points.
NPC's might react to this, and maybe instead of shouting out 'Oh, what strange garb' they shout 'Is that who I think it is?!'
it would be a IC way to reward people for their achievements, nefarious or altruistic, and acknowledge their success.

I play on a place in which a form of this is implimented. It is called Noteriety. You are required to earn this in addition to experience to gain levels. Noteriety also determines what size house you can buy and how soon you can rise in rank if you are in a faction. Clerics, druids, paladins who preach the more favored responses they get, the faster they can rise within the temple. It also factors in how much you have to pay informants to warn you or even stop assassination attempts. It also can factor into how many times the mysterious allies save you from death or you stop bleeding and do not die.
However you noteriety percent is hidden and only known to the admin of the world.

It is a good idea and I am sure if it is implemented there would be a standard by which all dms would adhere to as playing devils advocate

We have already made some inroads on this with our Immersion Tools (via "The Tome of Self" and matching DM tool aka "The Troublemaker"), but this is only the beginning.
With our present system you will note the 'known to me' factor, the more time you spend with a PC the higher this value becomes for your PC. Eventually, a running average of how many other PCs 'know your PC' will likely become the basis for PC fame. Other details about your PC are stored (when you use the Tome of Self) and can be discovered/divulged by others who know you (as rumors).
We need to flesh out the tools and for that, we do well to look at how those in D&D handle things - we'll borrow what ideas work and discard what do not (NWN is after all its own unique environment slightly different from traditional D&D by necessity).
